Father & Daughter Fatally Crushed by Container [Port Access Road, Sepanggar, Malyasia – 18 October 2026]
Norfaieza Mohd Jadi 53, and his daughter Mohd Jadi Pelbi, 24 were occupants of a late model Toyota Avanza while on a Sepanggar port access road on the evening of 18 October (last Sunday). Mr. Mohd Jadi had picked up Ms. Jadi Pelbi from work and was driving her home.
Concurrently, a 49 year old over-the-road truck driver was on the side of that road attempting to hoist an intermodal container onto his self-loading intermodal chassis. The container had apparently been placed on the side of the road earlier for future pick up.
As the car containing the two occupants was passing by and adjacent to the container transfer activity, the self-loading chassis and the container it was to load became unstable and tipped over. The entire rig fell directly on to the passenger vehicle, instantly killing the two individuals inside.
Many safety professionals serving the marine cargo handling industry have long expressed skepticism about the actual utility and performance of self loading chassis, while clearly understanding the circumstances wherein such equipment is deployed.
