Yard tractor operator Mr. Imam Kastiawan, 47, was fatally crushed by a fallen container while working in a container yard during vessel loadback operations at the PT Berlin Jasa Terminal Indonesia (PTBJTI) on 15 August 2022.
According to media reports (See links below), Mr. Kastiawan was in the cab of his tractor when a reachstacker loading a twenty foot container onto his trailer malfunctioned….. dropping the box directly onto his vehicle.
