In assembling several of the less-than-crystalline media accounts relating to this accident, Blueoceana Company is led to believe that the foregoing accident occurred aboard the Toll-owned vessel M/V TASMANIAN ACHIEVER (shown below).  When, and if, further details of this event become known, this post will be updated.

Update: 14 December 2016

News from Australia, is that employer/stevedoring company Toll Transport will be required to pay a one million dollar (AU) fine levied by a court of competent jurisdiction hearing arguments in relation to this very terrible accident.
