Withe great regret, we’ve been informed by a friend in South America about a fatal accident that occurred at the South Chilean port of Puerto Coronel on Wednesday (23 September).
In that accident, Sr. Patricio Octavio Martinez was killed when the mast of an industrial truck he was operating inside a marine terminal warehouse collapsed backward (See photos below) and crushed him within the vehicle’s cab.

Are there any specs on the equipment – age, mast height, make, model, etc.
work area information – weather, floor condition, etc.
Any other relevant information regarding the equipment or work area, would be appreciated.
The machine was a Hyster H360HD. I don’t know its age, nor its maintenance history

The indoor (warehouse) environment was reportedly unremarkable.
My understanding, is that Hyster recommended reinforcement of certain components near the mast connection.
